Students are blocked from registration by Student Health Services for the following:

• They fail to turn in a complete Health Record Form
• They have an outstanding balance at the health center

Students will be unblocked when the completed Health Record Form is turned into the Student Health Center and/or any outstanding balances are paid. These matters may be taken care of during normal hours of operation.
