AHPT 882:  Comprehensive Patient Care

Comprehensive Patient Care integrates clinical and basic science knowledge with skills acquired throughout the curriculum by evaluating patients with complex case presentations. Students will develop clinical reasoning skills for the successful evaluation and management of multiple systems within variable contexts to become culturally sensitive autonomous physical therapy practitioners. This course will emphasize the development of clinical expertise through reflective practice to integrate personal values with the best available evidence, and the analysis of outcomes to plan for lifelong learning. The format of this course uses a variety of learning experiences including lecture, problem based learning, simulation, small and large group discussion. Comprehensive Patient Care prepares students to participate in their third and final clinical experience.


Detailed Description of Course

Comprehensive Patient Care integrates both clinical and basic science knowledge and skills acquired throughout the curriculum by analyzing patients with multiple medical and physical diagnoses. Students will have the opportunity to apply problem-solving skills to the dynamic interaction of multiple systems (physiological, biomechanical, psychological, social, cultural, and environmental) and their impact on the disease and recovery process. Tests, measures, and interventions will be discussed related to the diagnostic categories described in the Guide to Physical Therapist Practice. The course prepares students to participate in their third and final clinical internship experience.
    

Detailed Description of Conduct of Course

Course content may be delivered by lecture, presentations by expert clinicians, practical on-site learning experiences, patient demonstrations, small-group cooperative learning, case study analysis/ presentations, supervised student/ patient interactions.


Goals and Objectives of this Course

Will require students to:

1) Analyze the relationship between co-morbidities and their effect on patient prognosis and treatment;

2) Select, prioritize and demonstrate the examination procedures (history, systems review and tests and measures) as applied to patients with multiple diagnoses, using the Guide to Physical Therapist Practice;

3) Analyze current scientific literature as it applies to patients with multi-system disorders;

4) Integrate physiological, biomechanical, pharmacological, psychosocial, cultural and environmental principles into the examination and treatment process of a  patient with multiple system disorders utilizing information gathered from multiple resources including medical and patient/ family resources;

5) Integrate knowledge of primary and secondary complications as they relate to the diagnosis, prognosis, and treatment of patients with multi-system disorders;

6) Identify appropriate long term and short term goals based on analysis of the interaction of a patient's problems with multi-systems disorders and resources available;

7) Design, prioritize, and implement a cost-effective treatment plan for a patient with multi-system involvement;

8) Identify the rationale for each intervention based on current scientific literature and standards of practice;

9) Describe and prioritize a plan of treatment progression;

10) Document findings and treatment plans including initial examination, daily progress notes, re-examinations, DC summary,  letters of medical justification and correspondence to other team or healthcare members using clear, concise, and appropriate language;

11) Design and teach an appropriate self-exercise program to improve the function, mobility and endurance of a patient with multiple diagnoses;

12) Demonstrate the ability to involve a patient with multiple diagnoses, family members, and other members of the healthcare team in the development of a plan of care;

13) Select appropriate community resources as they apply to patients' needs;

14) Prescribe and fit appropriate assistive devices and equipment as needed by each patient;

15) Identify communication issues and design a plan to facilitate and enhance both verbal and non-verbal communication;

16) Justify clinical decisions related to the PT diagnosis and plan of care of a patient with multiple medical diagnoses, based on current literature and standards of physical therapy practice;

17) Analyze and resolve ethical issues related to the resource utilization and treatment of patients with multiple diagnoses;

18) Identify lab value norms and their implications on patient function when lab values deviate from the norm;

19) Based on clinical observations and direct patient contact, develop and implement a PT examination, plan of care and outcome assessment for a given patient with multiple co-morbidities;

20) determine patient outcome measures through the use of a standardized tool.
